16. Take Survey Page When selecting the option will come as a popup asking for info. First popup will ask you to select facility visited. Must use check marks to select them. Once they are done they will need to select NEXT and move to page 2. On page two they will be asked for personal info such as age, grade and more. On page one we need to be able to add and remove activities. On page two data will be fixed to the values. For AGE, GENDER and more we need to be able to enter them in a drop down menu that we can edit. In managing survey we need the option to select a date range and print reports in .pdf sorting out various options. Example I want to know from Jan to Dec 2008 how many people aged 20 to 30 visits Outdoor Recreation. Or how many Rank NF-05 visit Auto Craft for 2008 and so on. Also needed multiple choices like all Gender: White Age: 20 to 30 and Community: Livorno visit Auto Craft in 2008. All reports need to be done in PDF and we need the option to save them to the computer or print them. Which MWR Services did you use the most in the past six months?
