SouthamptonCYPDS Service launching June 2012 • Education element includes the following professionals:Educational Psychologists, SEN Officers, Specialist Teacher Advisors, Portage Home Visitors, • Social Care element includes the following professionals:Social workers who are currently attached to working with children with disabilities and Complex Health Needs and Community Support Workers • The Health element includes the following professionals:Clinical Psychologists, Community Paediatricians, Community Support Workers, Continuing Care Nurses, LD Nurses, Occupational Therapists, Physiotherapists, Speech and Language Therapists Education Social Care Health 6

SouthamptonIntegrated Assessment (IA) and Single Plan (SP) Introduction • All children and young people meeting the CYPDS Eligibility Criteria will be offered the new IA and SP from June 2012 (approx 100 families from June 2012 – March 2013) • Single route of entry through ‘application’ to the CYPDS Decision Making Panel • The ‘application’ can be made by a professional, with parental consent or can be made by parents directly

  8. Southampton IA and SP Eligibility Criteria • 1. The child/young person meets the current criteria for statutory assessment of their special educational needs under the 1996 Education Act* or where the child is under 5, where notification has been given by Health professionals that they have difficulties that are likely to result in significant special educational needs in the future that might require a statement to be in place. • Or • 2. Where a child/young person has a significant learning disability and/or enduring complex health condition or impairment which significantly affects, or is predicted to affect, every day life functioning over the child/young person's development and complex family circumstances which affect the ability of the child/young person to reach their full potential. *The definition of SEN is as set out in the Education Act (1996):  ‘A child has SEN if his/her difficulties in learning  are significantly greater than those experienced by the majority of his/her peers and which prevent him/her from accessing the provision normally available in schools’ i.e.SEN are defined in terms of the interaction between the child/young person’s level of need and also the barriers that are preventing them reach their full potential. The assessment of level of need within this interactionist framework, is based on the child/young person’s presenting needs, rather than on any diagnosis which has been given. 8

SouthamptonRelationship with Universal, Targeted and Specialist Services • IA and SP will enable a multi-agency response from specialist services (Tier 3) & support services (Tier 2) • Support provided to professionals at Tier 2 where needed • Information gathered during IA can inform CAF process or be escalated to CP Plan without duplication 9

SouthamptonPersonal Budgets Introduction • Personal Budgets Pilots in Southampton will give families, children and young people more control about what education, health and social care services are used to support them. • Pilot projects that will run as part of the Pathfinder: • Continuing Health Care Personal Budget Project • Single Plan Notional Budgets with offer of some Direct Payments • Home to School Transport Direct Payments • Continence Direct Payments • The aim of the pilots are to ensure: • that Pathfinder families know and understand their funding allocation • its clear why that amount has been allocated • families are directly involved in deciding how funding is used

  14. SouthamptonWhat is a Personal Budget? A child or young person has a different budget for each service they access, for example: 1. Jigsaw Short Breaks: £6,000 2. Aiming High Short Breaks: £1,000 3. Physiotherapy: £2,500 4. School Support: £5,000 Collectively these become their Personal Budget Personal Budget A clear upfront allocation of funding to meet child/young person’s needs Choice of how the funding is spent Traditional services Direct Payments Or a range of options in between

SouthamptonContinuing Health Care Personal Budget Pilot • Health leading and monitoring as part of Southampton Personal Health Budget Pilot • Four children with complex health needs being supported • Funding sources include: Continuing Health Care Funding, Statutory Short Break Funding and Aiming High Funding • Health Lead Professional and Commissioning Manager supporting the family to influence how these budgets are best meeting their short break needs

SouthamptonSingle Plan Notional Budgets with offer of Direct Payments • Aiming to put a notional value to the Single Plan – costing as many elements as possible • Lead professional will work with the family to identify where there are opportunities to take funding as a direct payment (where available) • Systems being developed to support families and develop the market place to offer alternative provision in regards to short breaks • Next Steps include: • Further developing the processes to enable families to have more choice and control in how they receive their short breaks • Increasing the number of direct payments given to families who receive short breaks

SouthamptonHome to School Transport Personal Budgets • The aim is to offer families the choice: • A personal transport budget as a direct payment or • Continue using traditional school transport from January 2013 • Two Special Schools are to be part of the pilot • Next steps: • Financial modelling to work out the unit cost calculation and understand the impact on current contracts • Consultation with parents and other stakeholders • Sign up of families to the pilot and roll out project

SouthamptonContinence & Equipment Personal Budgets • Exploring the possibilities of offering a personal budget to families that currently receive continence products • PCT undergoing Any Qualified Provider (AQP) procurement process which will open up the range of providers that provide continence products • Families to receive direct payments to use with AQP • Exploring the possibilities of offering a personal budget to families that currently need equipment • Funding for low cost items could be offered as a voucher to families so they can purchase themselves and top up funding if felt appropriate • Development work to start from September 2012

SouthamptonVulnerable Children • The scope of the integrated assessment and single plan: What will the eligibility criteria be for the defined group of vulnerable children to receive an integrated assessment and single plan. What services will need to work together, strategically and at case level. What impact will this have on the workforce? • Better support and outcomes for children: What will be the added value of the defined group of vulnerable children having an integrated assessment and single plan? What difference will it make for their outcomes? • Working cross-boundary: How can could an integrated assessment and single plan better support children who live and/or are educated in a different local authority to the local authority that is responsible for their care? How can we support better communication across local authority and PCT cluster boundaries? 19
