Southampton SEND PathfinderOne and Only Pack • As far as possible, there should be a ‘tell us once’ approach to sharing information, so families and young people don’t have to repeat the same information to different agencies, or different practitioners/services within each agency.
Southampton SEND PathfinderKey Working: Assessment Co-ordinator • Supported through the assessment process by a single point of contact • Children, families and young people are at the centre of the assessment and planning process and should be partners • The assessment process should be co-ordinated, person centred, timely and without duplication.
Southampton SEND PathfinderKey Working: Lead Professional • Builds on previous assessments and continues to identify and meet the needs of children, young people and families as they develop • Annual person-centred reviews • Practitioners should be engaged and committed to the co-ordinated assessment and planning process and where necessary, trained in person centred approaches.
Southampton SEND PathfinderEducation, Health and Care Plans • EHC plans should be person centred, evidence-based and focussed on outcomes • EHC plans should support preparation for key transition points. Plans must be “forward looking” • EHC plans should explore how informal (family and community) support as well as formal support from statutory agencies can be used to achieve agreed outcomes
