  4. Define • Strategic Plan Goal- By 2013, every Brevard school will serve every student with disabilities in an inclusion model, in general education classes with access to the general curriculum, for at least 80% of their school week, unless the IEP clearly states how the nature or severity of the disability is such that education in regular classes with the use of supplementary aids and services cannot be achieved satisfactorily.

  5. Define • Which Q measures will be improved? Q2 - # of referrals meeting the 60 day DOE requirement Theme Statement: 100% of initial ESE referrals will meet the DOE 60 day timeline

  6. Measure • Potential problem areas and associated • P measure(s) for each: • P1 No data collected on the # of Informed Notices Provided to parents • P2 100% of referrals are not filed in the area office(s) within the district 10 day timeline • P3 100% of evaluations are not completed within the 60 day DOE timeline Most significant problem: What? Incomplete/ poorly prepared paperwork When? November 2008 Gap? The DOE 2007- 08 results indicate that 94% of referrals in Brevard met the 60 day timeline.

  7. Analyze • Identify and verify root causes from problem identified in “measure” step • Little understanding of consequences or urgency by school personnel • School based personnel do not see this process as a priority • Lack of adequate staffing at schools • Incomplete paperwork received by area offices • Paperwork directions are unclear

  8. AnalyzeApproach used to select root causes • 5 Why’s Discussion with school and district based staff • Deadlines not met • Observation, staffing plans • Incomplete and sloppy • Incorrect paperwork and requests for help • Urgency not understood by school personnel • Process not prioritized by school personnel • Increased staff responsibility due to inadequate coverage • Missing or incorrect information on paperwork • Directions are too general

  9. Improve

  10. Improve

  11. Control How will selected root causes identified in Analyze step be reduced or eliminated? Both the 10 day district timeline and 60 day DOE timeline will be met at 100%. How much will the problem from the Measured step be reduced? The gap will be reduced by 6%, meeting the 100% target requirement. How much will the theme from the Define step be affected? Theme: 100% of initial ESE referrals will meet the DOE 60 day timeline. Root causes will be eliminated. Data will reflect compliance.
