  2. Enrolling in AFL Benefits • 30 days to make an election for 2012 benefits. • New Hires will receive Benefits are effective on the 1st of the month after date of hire. If you are hired on the 1st of the month they are effective on your date of hire. • New hires have an enrollment worksheet in the mail within 10 days of hire. • Review the Benefits Enrollment Guide & the Enrollment Worksheet. Make your Decisions • Enroll On-line on the AFL Benefits Enrollment Site from the Internet or AFL’s Intranet or by phone. Your default password is two digit birth month and birth year (MMYY of your DOB).

  3. Enrollment • If you fail to enroll in medical, dental, vision, and FSA, plan coverage will be waived and you will not be able to enroll in those plans until 2013 unless you experience a family status change or life event.

  6. Dependent Requirements • Eligible Dependents: • Legal Spouse • Children to age 26. (Biological, step-children, adopted, court-ordered). • AFL may deny coverage if child is eligible for another group plan (grandfathered plans) • Dependents beyond these age limits who are unable to care for themselves due to a physical or mental disability.

  7. Enrolling Dependents • Social Security numbers and birthdates (including the year) are required for all dependents. Please ensure you have that information available when enrolling • Spousal Surcharge (medical plan only) – Must complete spousal employment questionnaire

  11. Vision Plan • No ID card required • Co-payment for annual eye exam: $10 • Co-payment frames: $20 $120 allowance towards frames or contact lenses

  12. Short- and Long-Term Disability • Short-term • Basic coverage: 60% • Buy-up option: 80% or 100% • Long-term • Basic coverage: 50% • Buy-up option: 70%

  13. Life Insurance and AD&D • Basic Life for you: one times annual salary • Buy up: one to 5 times your annual salary • optional life for you, your spouse and dependents • Basic AD&D: one times annual salary • Buy up: one to 5 times your annual salary • optional AD&D for your family

Flexible Spending Accounts • Health Care – allows you to contribute pre-tax dollars to pay for a variety of out-of-pocket medical, dental, Rx, vision expenses not covered by any other plan. • May contribute $100 to $3,000 per year • Dependent Care – allows you to contribute pre-tax dollars into an account to pay for qualified dependent care expenses • Up to $5,000 ($2,500 if married filing jointly) • Flexible spending dollars must be used or they are forfeited.

AFL Benefits Voluntary Critical Care The Voluntary Critical Care plan is designed to help employees offset the financial effects of a catastrophic illness with a lump sum benefit ($5000 or $10,000 for Employee, $5,000 for Spouse) if diagnosed with a covered critical illness Covered conditions include: -Heart Attack -Coma (as a result of severe traumatic brain injury) -Cancer -Benign Brain Tumor -Stroke -Permanent Paralysis (as a result of a covered accident) -Blindness -Carcinoma in Situ (25% of lump sum benefit) -Major Organ Failure -Coronary Bypass Surgery (25% of lump sum benefit) -End-state Renal (kidney) failure -Occupational HIV Children are automatically covered at 25% of the employee election and are included at no additional charge as part of the employee benefit. Child conditions also include: -Cerebral Palsy -Cleft Lip or Palate -Cystic Fibrosis -Down Syndrome -Spinal Bifida Each condition is payable once per lifetime per covered individual. Additional reduced recurrence benefits are payable for diagnosis of another critical illness if separated by 90 days or more and medically unrelated.

  16. After Enrollment • You will receive a Benefits Confirmation Statement at your home and this is the final opportunity to make any changes • New ID cards will be sent to you • HealthCare. • Prescription, • Dental • FSA, your debit card has an expiration date printed on it.

  17. Making Changes • Birth or adoption • Death • Marriage • Divorce • Employment status – Gain or lose coverage Must be completed within 31 days of the event Go the AFL Benefits Enrollment website

AFL 401k Savings Plan • New Hires are eligible to participate as soon as practical after date of hire. • AFL’s plan is administered by BB&T • New hires will receive an enrollment packet 15 working days after date of hire. • AFL offers employer match of up to 4% ($ for $ first) • AFL has a 1 year vesting period

  19. Employees can defer pre-tax dollars up to 75% of earned wages. Pre-tax deferral are limited by IRS limit of $17,500 for 2013 Employees can also elect to defer after-tax dollars Employees can also elect to defer up to 10% of any bonus received Employee can make changes to deferral percentages at anytime via the website or through the telephone system.
