  1. Severity 4 & 5 Observations Dec. ’2010 East Region

  2. Total severity 4 and 5 observation for the month of Dec.’10:- 17 Nos.

  3. After Severity 5- One crane released from assembled rafter without proper support & communication.( LD#3) (Model-II) _East . Action Taken:- Provided proper supports and released crane sling.

  4. Severity 5- Four workers came at a place in between two lifeline posts for shifting of purlin.( LD#3) (Model-II) _East. Action Taken:- Activity stopped and purlin shifted by crane as weight is more.

  5. Severity 4- Welding cable was being extended by using holder. (Cable Jointer not used)( LD#3) (Model-II) _East. Action Taken:- Activity stopped and removed the extended cable.

  6. Severity 4- Four workers came in boom-lift bucket . .( LD#3) (Model-II) _East_Jharkhand. Action Taken:- Training imparted and JCC(Job Cycle Change) done for those workers and boom lift operator.

  10. Severity 5- Lifeline not fixed properly at the eave of roof of low bay area.(TGW, Indor., J S Enterprises) _West- Maharashtra Action Taken:- Lifeline at eave side provided.

  11. Severity 5- Electrical board were used without ON-OFF switch & No ELCB.(ChurchGate, Mumbai – Sai Project ) _West- Maharastra Action Taken:- Electric Board with ELCB mobilized.

  12. Severity 5-Leakage Found in hydraulic jack of Crane. (Church gate, Mumbai – Sai Projects) _West- Maharashtra Action Taken:- Corrected.

  15. Severity 4:- Full body Safety harness without Shock absorber & Single Lanyard, snap hook instead of Scaffolding Hook being used. ((Siemens Mundra – C.P.P.L.) _West- Gujrat Action Taken:- Safety harness with shock absorber with double lanyard provided.

  16. Severity 4:- Two Riggers found working by standing on Girt . ((Siemens Mundra – C.P.P.L.) _West- Gujrat Action Taken:- Activity stopped and Boom Lift used for the same activity.

  17. Severity 5- Painting Compressor found without guard .(Siemens Mundra – C.P.P.L.) _West- Gujrat Action Taken:- Instruct Builder to provide Guard immediately.

  18. Severity 5- Gas Cutting cylinder found on road & valve found bend.(Siemens Mundra – C.P.P.L.) _West- Gujrat Action Taken:- Cylinder was removed from site.

  19. Severity 4:- Gas Cutting Hose found Damage & joint found in between .(Siemens, Mundra – C.P.P.L.) _West- Gujrat Action Taken:- Damaged hose removed from site.

  20. Severity 5- In Gas cutting set Flash back arrestor were used only at torch side ..(Jindal SS, Savli, Vadodara - R.K. Builder) _West- GUJRAT Action Taken:- Activity stopped immediately and flash back arrestor provided.

  21. Severity 5- Scaffolding were used without locked wheel and cross bracing.(Jindal SS, Savli, Vadodara - R.K. Builder) _West- GUJRAT Action Taken:- Activity stopped and proper scaffolding provided.

  22. Severity 5- Electrical board were used without ON-OFF switch.(Jindal SS, Savli, Vadodara - R.K. Builder) _West- GUJRAT Action Taken:- Activity stopped and electrical board replaced.

  25. Severity 4 :- Worker found walking on the rafter where there is no fall protection systems ( Life line Post and lifeline)– (Autoliv site. )_AP&K Action Taken: Activity stopped & life line provided.

  26. Severity 4 :-Area not barricaded below where height work is in going on. (Autoliv site)_AP&K Action Taken: Activity stopped work & area barricaded.

  27. Severity 4 :- Life line post and lifeline not provided properly- (Autoliv site)_AP&K Action Taken: Life line post & life line provided as per TBSL standard.

  28. Severity 4-:-Vehicle movement under suspended load. (Auto liv)_AP&K Action Taken: Instructed builder personnel barricade the area to avoid below movement

  31. Severity 4-: Hydra wire rope damaged – (Rock Worth)_TN Action Taken: Activity stopped and hydra sent out .

  32. Severity 4-: Life line rope has joints – (Rock Worth)_TN Action Taken: Entire Life line rope Replaced.

  33. Severity 4-: Poor Housekeeping & Unhygienic woking condition – (SCL Site)_TN Action Taken :Instructed builder to Remove all unwanted insulation material - Closed

  34. Severity 4-: Un safe scaffold – (SCL Site)_TN Action Taken : Rectified & safe access provided- Closed

  35. Severity 4-:Inproper Electrical Practise – (SCL Site)_TN Action Taken : Informed customer to proved proper electrical supply - Not Closed

  36. Severity 4-:Improper material staking – (SCL Site)_TN Action Taken : Housekeeping done.

  37. Severity 4-:Building erected With out any life line rope –(Sinduja site.)_TN Action Taken : Stopped all activity & issued warning letter

  38. Severity 4-: Mobile scaffold erected wheel have no locks –(Sheenlac Site)_TN Action Taken : Given proper training and immediately removed all unsafe scaffold.

  39. Severity 4-Worker found standing on the girt to fix the next level girt. Action Taken: Proper scaffolding with working plat form to be used to work at height. Work stopped scaffolding moved from other side.

  40. Severity 4 & 5 observations Dec. ’2010 North Region

  41. Total severity 4 and 5 observations for the month of Dec.’10:- 05 Nos.

  42. Severity 4-: Limit switch of hydra found not in working condition & also safety latch found damaged, (Siemens, Mohindergadh ) Action Taken: Stopped use of equipment & corrected
