Innovation, Excellence and Strategic Development (IESD) Fund The funding programme has: 3 strands Innovation, Excellence & Strategic Development 5 themes • Personalisation & Choice of care and support • Delivering better health and care outcomes • Improving public health • Improving long term care and support • Delivering safe and compassionate care And various priorities within each theme
Health & Wellbeing Visits project responds to The innovation strand - offering a new creative approach to improving health & wellbeing The delivering better health and care outcomes theme to support improved health & care outcomes including interventions that will result in measurable reductions in health inequalities and improvements in health and life expectancy in disadvantaged groups Promoting prevention and early intervention approaches
Health & Wellbeing Visits project Aims to reduce health inequalities and increase life expectancy for vulnerable older people living in deprived neighbourhoods
Who is the project for? Older people in East Sussex and Brighton & Hove who may be vulnerable By this we mean: People over 50 with disabilities or long term conditions People over 65 who • face social exclusion • live in communities or groups facing health inequalities • live alone • are income deprived People over 80
What are we trying to achieve? Older people at higher risk of health deprivation will be: more aware of their health needs and their choices of local public & community services taking more action to manage their health better more socially connected
How will we achieve this? • East Sussex Fire & Rescue Service Community Volunteers will support older people to: • assess their health and wellbeing needs • understand their choices • access services that meet their needs
Assess their needs Community Volunteers will visit people in their own homes to talk about their health & wellbeing; providing support to assess and understand their own health needs. thinking about mental & physical health nutrition social isolation substance use winter warmth communication needs mobility general welfare health & safety sight issues
Their Choices Based on any health & wellbeing needs that have been identified, people will be made aware of their choice of services in their local area Public services Voluntary services Community and support groups
What else we will monitor? We will invite local commissioners to join our Learning & Development Group and ask them to tell us what impact indicators they want to see as evidence. This might include: • Hospital admissions for preventable health conditions • People able to live at home longer • GP visits for advice
Learning & Development Group We aim to engage a group of commissioners to advise the project on what specific impact indicators they want to see as evidence, that will induce them to commission the intervention in the future • Adult Social Care • Public Health • Clinical Commissioning Groups To build trust and ensure good quality evidence, impact towards these indicators will be independently evaluated We hope other stakeholders will also be engaged to advise and champion the project
SharingExcellence We aim to have an impact nationally by sharing learning and supporting implementation in other areas. We will use these channels to help achieve this: • NAVCA & NCVO • Chief Fire Officers Association • South East Emergency Services Volunteer Coordinators Forum • South East Regional Voices
