  1. PROCEDURA INSERIMENTO AI SENSI DELL’ART. 57DEL REGOLAMENTO REGIONALE 4 DEL 2007 COMUNITA’ SOCIO RIABILITATIVA (“DOPO DI NOI”)

  2. Informazione del Servizio Sociale del territorio di residenza dell’Ospite e produzione della documentazione clinica del disabile e del modello ISEE per valutare la capacità di reddito.

  3. Contatto e condivisione con il Medico Curante a riguardo della volontà della Famiglia, dell’Ospite o del Tutore circa l’inserimento H 24 in struttura dell’interessata/o.

  4. Compilazione della SVAMA da parte del Medico Curante

  5. Deposito presso il Distretto Socio Sanitario di residenza del modello di richiesta di inserimento e della SVAMA compilata dal Medico Curante

  6. Valutazione da parte della UVM territoriale e autorizzazione all’inserimento presso la struttura adeguata al setting socio assistenziale e sanitario del caso in esame

  7. Il Distretto invia alla struttura il modello di autorizzazione firmato dai componenti della UVM con indicazione della retta

  8. SPIEGAZIONE DELLA RETTAE RELATIVA RIPARTIZIONE

  9. RETTA COMPLESSIVA € 64,38 QUOTA A CARICO ASL SEMPRE 40% PARI A EURO 25,75 QUOTA A CARICO COMUNE 60% PARI A EURO 38,63

  10. La quota del Comune del 60% può essere invece a carico dell’Utente in base alla valutazione ISEE svolta dai Servizi Sociali competenti sul territorio di residenza dell’Ospite, oppure Può essere a carico di entrambi (percentuale a carico Comune e percentuale a carico Utente) in base ai regolamenti dei comuni in vigore che stabiliscono la soglia per ottenere la partecipazione alla retta, sempre in virtù del modello ISEE
