    1. Third Party Fatality INCIDENT DETAILS: Date & Time : 05th January 2006 @ 12:00 approximate Location : Muscat/Salalah Main Road near Al-Agayez Area Actual Severity : 4P Potential Severity : C4P Contractor/ Sub-contractor : BHEL/ Al-Hassan Third party : driver Fatality passenger LTI (broken skull) passenger MTC Contractor : driver LTI (broken foot) 2 passengers MTC. DESCRIPTION OF THE INCIDENT: Al-Hassan Engineering Co. vehicle (AHEC) had a head-on collision with a third party vehicle along the NIZWA-SALALAH highway approximately 115 kms from Qarn Alam towards Haima. The AHEC truck was travelling to Haima with 2 passengers. The third party saloon car was travelling to Muscat with 2 passengers. The 3rd party driver died in the hospital on 13th January 2006. Other third party occupants and AHEC driver received various degrees of injuries. The four IPs were immediately transferred to Haima hospital and later to Nizwa Referral Hospital. The two AHEC passengers received slight first aid injuries.

Failure in implementation Quality and effectiveness of Journey Management audits Driver pool and fleet management Journey authorization Vehicle allocation Monitoring of vehicle maintenance and certification HSE Communication Not all drivers attend the relevant HSE Meetings / Forums targeting Road Safety / Hearts and Minds. Follow up on action items HSE Advisor identified RAS certification expiry in 6 weeks but no action instigated Roles and Responsibilities Mechanic and driver both did not check tyre pressure Journey Manager, driver, and mechanic did not pick up RAS expiry
