Group Life Insurance Rates No change to Basic and Dependent rates Age-Based Supplemental Rates • Pay period that includes April 1, 2008. Going Forward • New employees - age on the effective date of coverage. • Adjusted each April 1 to reflect: Level: any change in gross compensation for previous calendar year Age: any change in rate band, based on age
Age Based Supplemental Rates 40 Year Old Employee with Basic and Supplemental 3 coverage and a $42,000 level.
Grandfathered Employees Have supplemental coverage on April 1, 2008. Teacher employees 55 or older - rate remains at $.10 per thousand bi-weekly. PLD employees 65 and older - rate remains at $.21 per thousand bi-weekly. Rates remain in effect for the employee as long as the employee remains with current employer. Age-Based Supplemental Rates

Evidence of Insurability • Evidence of Insurability –how an eligible employee may request new or additional coverage. • If approved, coverage is effective the first of the month after an additional 31 days in an eligible position. • Employer notified of new effective date and coverage code. • Do not collect increased premiums until the period which includes the new effective date.
Evidence of Insurability • Employee makes initial request to MainePERS by form, phone or e-mail. • MainePERS sends employee a form requesting medical information, which employee sends to the carrier for review. • Process usually takes from 6 to 12 weeks • All further communication is between employee and insurance company.
Designation of Beneficiaries • Claims paid based on most recent beneficiary form postmarked or received at the Retirement System prior to the death of the member. • Members encouraged to review designation every 5 years, or when they have a life changing event. • If no designated beneficiary: payment to first living of the member’s spouse, children, parents, or to their estate.
Chapter 601 • Employee does not have coverage unless he/she submits form: “Application for Coverage – Group Life Insurance” • Teachers who change employers are treated as new hires; within the first 31 days employees may select any coverage • Full and part time employees must work at least 20 hours or 2.5 normal working days per week to be covered
