NEW NEURO/CARDIO aPTT TARGET RANGE & NOMOGRAM

1. NEW NEURO/CARDIO aPTT TARGET RANGE & NOMOGRAM EFFECTIVE TUESDAY MAY 26TH 2009 AT MIDNIGHT

2. The Hematology Lab is installing new equipment to measure aPTTs. The new equipment will affect SBUMC aPTT values. • This change will impact the Neuro/Cardio IV Heparin Protocol Target Range. Currently the target range for aPTT is 50-75 sec. • The NEW Target Range for Neuro/Cardio is aPTT 60-85 sec. • The DVT/PE Heparin Protocol WILL NOT CHANGE. Target aPTT will remain 65-90 sec. for DVT/PE Cont. to use current forms for DVT/PE

3. What Will Change • Heparin Neuro/Cardio Flow Sheet Nomogram (New Flow Sheets required) • IV Heparin Neuro/Cardio Order Set Nomogram ( New Order Sets required for EPR downtime ordering) • EPR: Neuro/Cardio Heparin Nomogram

4. What you need to do on your Unit • Remove all current IV Heparin Neuro/Cardio Order Sheets (dated 6/08) and Infusion Flow Sheets (dated 10/08) • Replace with New IV Heparin Neuro/Cardio Order Sheets and Infusion Flow Sheets ----They will be printed in PINK for easy identification. aPTT target range will be 60-85 sec. • Train/Notify Staff: • New Target aPTT range of 60-85 sec. • Use only PINK IV Heparin Neuro/Cardio Flow Sheets • Use only PINK IV Heparin Neuro/Cardio Order Sheets ( for EPR downtimes) • DVT/PE IV Heparin Protocol will NOT CHANGE!! Continue to use current forms • Change will take effect on Tuesday May 26th at MIDNIGHT
