Lehigh Valley Health Network – AIDS Activities Office In+Care 04 – Viral Load SuppressionPercentage of patients, regardless of age, with a diagnosis of HIV/AIDS with a viral load < 200 copies/mL at last viral load test during measurement year AAO Goal VL <200 = 80% Interventions specific to individual clients needs that are in the Adherence Program** • Will help pay co-pays for clients • Provide transportation assistance – bus passes or taxi vouchers • Frequent adherence appointments will be scheduled as needed • Frequent follow up phone calls • **The Adherence Program • The adherence program is a “revolving door”. The sicker clients are enrolled in the program and those that improve graduate from the program. • These clients have multiple health issues, i.e. diabetes, mental health, high blood pressure to deal with along with their HIV and have additional medications to take • The adherence program will continue to help clients understand how to take their medications and the importance of staying on their medications for a better quality of life. • 2012 Adherence Program Results: % of clients with • VL <200 • 1st quarter - 63% (58/92) • 2nd quarter – 62% (53/86) • 3rd quarter – 49% (41/83) • 4th quarter – 63% (70/112) Baseline: 2011 – 74% (528/711) VL <200 Interventions – Started 2012 • Run monthly reports with VL’s for adherence nurses to monitor • Posted a bulletin board for visual reinforcement in the clinic on the importance of medication adherence • Provider alert sent to adherence team to for >200 • Have different colored “pre-packed pills – color for each time of day – unable to do • Utilize new Outreach Liaison to clients lost to follow-up – April 2012 added (resigned September 2013) • Improvement VL <200 – All AAO Clients • 2011 – 74% (528/711) • 2012 – 76% (554/725) – (1 of 2 adherence nurses transferred) • Jan – Sept 2013 – 81% (625/768) (2 adherence nurses)
