Community Outreach Health Educator • Indian Health Board of Minneapolis INDIANHEALTHBOARDOFMINNEAPOLISJOBDESCRIPTION COMMUNITY OUTREACHHEALTH EDUCATOR REPORTS TO:_DEPARTMENT:_DATE:_APPROVED:_ JOBSUMMARY This positionexiststosupporttheDiabetes EducationProgramandtheSDPIDiabetes PreventionProgramthroughthedevelopmentandimplementationofdiabetesanddiabetespreventionhealtheducationwithinIndianHealthBoard’s Clinics andinthecommunity. ESSENTIALFUNCTIONS Provideindividual,groupandcommunityeducationandoutreachforpeoplewithorathighriskfor diabetes,and maintainaccuratewritten recordsofindividualandgroupservices. Assistdesignand/orimplementationofdiabetes,diabetespreventionandcomplicationsprevention educationcurriculum,communityscreeningsandidentificationofhighriskindividualsutilizing programprotocols. Assistinthecreationofdiabetesprevention,self-careandcomplicationpreventionhealtheducationmaterialsasneeded. Providehealtheducationinthesurroundingcommunity (TwinCities). CollaboratewithotheragenciesandcommunityresourcesandrepresenttheIndianHealthBoardat meetingsandwithotherorganizationsas appropriate. Workwithprovidersandothermedicalstafftoaddressareasofgreatestneedfordiabetes-related healtheducationwiththeIndianHealth Board’spopulation. Assist inmeeting regulatorystandardsregardinghealtheducationofsuchorganizationsas JCAHO. Participatein Quality Improvement activitiesasneeded. Performother job-relateddutiesassignedbysupervisor. MusthavevalidMinnesotadriver’slicense,withnomovingviolationsinpast36months,andbe availabletoworksomeeveningandweekendhours. PLANNINGACTIVITIES Assistindevelopingand/orimplementingdiabetesanddiabetespreventionprogramsthroughparticipationinDiabetesTeam meetings, communitysurveysandsmallpilot studies. Participateindepartmentalplanningeffortsinordertomeetdepartmentalandorganizationalgoals andmaintainstandardsofcare. Maintainknowledgeofcurrentandculturallyappropriatehealthcarepracticesanddiabetescareand prevention recommendations. Understandcurrentrequirementsofgrant andfundingentity(ies). IMPLEMENTATIONACTIVITIES Activelyinvolvedinplanning,implementationand reportingofdiabetesgrantactivitiessuchascommunityclasses,diabetesself-managementandpreventionprogramevents,exerciseactivities andDiabetes/MetabolicSyndrome/Pre-diabetesdatabase. Implementthemeasurableobjectivesofgrantsandstandardsofcareinconcertwithotherstafftoaddressthehealthneeds ofIHB’s clientpopulation andanytargetedpopulations. ProvidehealthcareactivitiesinaccordancewithIHB’smissionstatementandgrantedagreements. Assist inmeeting regular standardsofBPHC, JCAHOandHIPPA.

  2. EVALUATIONACTIVITIES Participateinperformanceimprovement activities,as needed. Proficiencytestingconductedonanannualbasis, toevaluateskillsrequiredforthisposition. Monthlyproductivityevaluationtoassesstheeffectivenessof meetingclinic’smeasurableobjectives. Quarterlyperformanceimprovement reportstoassesstheeffectivenessofthedepartment in meeting measurableobjectivesofthecontracts. KNOWLEDGE,SKILLSANDABILITIES Excellentwrittenandverbalskills. Provenability toprovidehealtheducationtobothindividualsandgroups. Culturallysensitiveofdiversepopulations. Must beabletoworkindependentlyandasateam member. KnowledgeandrespectoftheAmericanIndiancultureandotherculturallydiversepopulationsweserve. EDUCATIONANDEXPERIENCE Atleasttwoyearsexperienceprovidingdiabetesandrelateddiseasehealtheducation,especiallyincommunitysettings. CertifiedDiabetesEducatoris optimal. Computerskillshelpful.KnowledgeofAccessdatabaseanddesktoppublishinghelpful. Continuingeducation requirementsasneededto maintainlicenseand/or certification. HIPAA IHB-Mmakesreasonableeffortstolimit accesstoanduseofprotectedhealthinformation (PHI)by employeestotheminimumnecessaryperformanceofassigneddutiesasoutlinedinjobdescriptions. ThispositionisaccessrestrictedtoPHI neededtoprovidetreatment inassignedarea PHYSICALDEMANDS Theworkrequiressomephysicalexertionsuchaslongperiodsofstanding,walking,andrecurring bending,crouching,stooping,stretching,reaching,orsimilaractivities.Theremayberecurringliftingof moderatelyheavyitemssuchasrecordboxes.Theworkmayrequirespecific,butcommon,physical characteristicsandabilitiessuchasaboveaverageagilityanddexterity.
