Join the Coached Care for Diabetes Project, a collaboration between the University of California, Irvine and patients with diabetes from local communities. This innovative project focuses on improving diabetes outcomes by identifying and addressing barriers to diabetes management. Our trained coaches work with patients before doctor visits to identify key issues, and help patients communicate effectively with their healthcare providers. Measures of success include laboratory data analysis, patient questionnaires, medical record reviews, and provider surveys. Diabetes is a community problem that requires a community approach.
