2012 Base Coverage • $0 monthly employee only deduction (Legacy & Horizon Employees) • $ 1800 single Health Deductible, $3000 family • Must meet $1800/$3000 deductible before receiving pharmacy benefits • 2012 Select Coverage • Legacy $20 monthly • Horizon $38 monthly - employee deduction for employee only coverage • $ 1000 Health Deductible • $75 Pharmacy Deductible
State Plan Changes - 2012 • Certification Requirement for CTs and MRIs in Emergency Room Setting Removed • Additional certification requirements for transplant evaluations with Active Health in addition to regular transplant pre-certification • HealthQuotientSMRequirement for Medicare Participants – if Medicare Primary then no Health Quotient required; if on Medicare but it is not primary then a Health Quotient is still required. • Participating Pharmacies to Administer Certain Vaccines under Wellness Benefits like flu shots • Weight Management Program through Active Health • Premium Changes for Retirees only – no changes for active employees • It’s not too late to complete Health Quotient for 2011 and use wellness benefits go to www.webmdhealth.com/mississippi
