You always have to give your SIS-card to the hospital • Hospitalisation expenses - in one of the hospitals accepting the AssurCard You have no payments to make to the hospital. All payments are settled directly between the hospital and AGF.AGF will recover the portion that is not covered; the expenses related to comfort and luxury, …

Offered by NATO Hospitalisation - in a hospital NOT accepting the AssurCard You have to fill out, date and sign a form “Declaration in case hospitalisation or serious illnesses” and send it directly to: Wyckaert – ComaritTav Sophie BrackeAfrikalaan 2879000 GENTsophie.bracke@wyco.be We advise you to pay first the bill to the hospital, then send the original invoice to Wyckaert-Comarit together with the prove of payment so that AGF will reimburse you. Please also send all the full details of medical fees, the mutual health fund receipts with the details of the legal intervention, full details of the pharmaceuticals prescribed by the doctor or any other documents evidencing the payment of special costs.

Offered by NATO Hospitalisation • Ambulatory expenses during the pre/post hospitalisation period or in case of serious illness • Make a copy of all medical documents and invoices • Mail the original medical documents and invoices to your mutual health fund and the copies to Wyckaert-Comarit, Afrikalaan 287 te 9000 GENT (preferably with the statement of reimbursement of your mutual health fund). • If there is no intervention by your mutual health fund, please sent all the original documents to Wyckaert – Comarit.
