Corporate Card Application Guide • ‘AMEX Corporate Card Application Form’ can either be obtained from HR department (hard copy); or at the link on the Travel web: http://corp.americanexpress.com/gcs/clients/hp/region/docs/japa/MalaysiaCardApp.pdf. Please note that you can complete the web version on screen and then print it for signing and sending. • Complete section 1 to 5 of the application form in full (“Employee ID’ is mandatory information required by HP) • Read the declaration at section 6, sign and date. • Attach the application form with your photo-copy of I/C (front & back) for Malaysian; or photo-copy of passport & work permit for Foreigner. The documents to be certified true copy by the authorized signatory and company stamp. • For Expatriates - Passport Copy - signed off by the authorized signatory and Company Stamp • For Expatriates - Work Permit - signed off by the authorized signatory and Company Stamp • Scan and email completed form & supporting documents to NazuraHamdan, (nazura-binti.hamdan@hp.com) HR • Department for verification and approval. HR will issue a letter confirming your position and income to Amex. • Items to note: • Turn around time for your Corporate Card Application is 15 working days upon receipt by American Express; missing or inaccurate information may lead to delays. Travelers are encouraged to apply their corporate cards early. • Application status enquiries can be made to Amex Telephone Service Centre at 1-800-88-9559 (8.45am to 5.45pm, Monday – Thursday; 8.45am to 4.45pm, Friday). Amex will only be able to advise status once the application has been received from your Program Administrator.
