Corporate Card Application Guide • Complete the ‘AMEX Corporate Card Application Form’ via New Zealand URL Link on the HP Travel website (http://corp.americanexpress.com/gcs/clients/hp/region/docs/japa/NewZealandCardApp.pdf). • Please note that you can complete the form on screen and then print it for signing and sending. • Guideline for Section 3: • Please only fill in your ‘Employee ID’ and leave the rest of the fields blank • Complete Sections 4 & 5 in full. • Read the declaration at Section 6, sign and date. Fill the name of the Certified Signatory engaged by you to certify your photo ID proof and proof of residential address. • Section 7 is to be left blank and will be updated by the APJ Amex DAR (Dedicated Account Rep) before sending it for processing. • In order to comply with the Anti-Money Laundering and Countering Financing of Terrorism Act 2009, American Express now requires ALL new Corporate Card applicants to provide primary identification documents with Corporate Card application forms to verify the applicant’s name and residential address. • All new Card applicants must: • 1. Provide a certified copy of a primary photo identification document. The certification must not be over three months old. Primary identification documents include: current Passport or current NZ drivers license. ( Please refer page 3 to know more about Certification : MUST READBEFORE SENDING THE APPLICATION FOR PROCESSING) • 2. Provide proof of their residential address. Proof of address means a copy of a utility bill, bank statement or Government-issued document or letter, less than three months old. • When completing new applications, please note you must submit supporting documents which establish your identity in the following way: • Engage an external Certified Signatory*, who must authenticate a copy of the original photo identification document as a true copy • 7. Upon completion, please scan the signed application form and copy of the certified photo ID proof and email to apjcontactamex@hp.com for processing.
Corporate Card Application Guide • *Certified Signatory • This is an individual who has been defined under the ‘Act’ as having authority to sight and certify a copy of an identification document as being an original • copy of that identification document. The following persons are Certified Signatories OR you may refer to the below URL to find a Certifying Officer near your place • http://www.jpfed.org.nz/Links.html • Commonwealth representative (as defined in the Oaths and Declarations Act 1957) • An employee of the Police who holds the office of constable (as defined in section 4 of the Policing Act 2008) • • Justice of the peace • • Registered medical doctor • • Kaumátua • • Registered teacher • • Minister of religion • • Lawyer (as defined in the Lawyers and Conveyancers Act 2006) • • Notary public • • New Zealand Honorary consul • • Member of Parliament • • Chartered accountant (within the meaning of section 19 of the New Zealand Institute of Chartered Accountants Act 1996). • Please note: The Certified Signatory cannot be related to you, be your spouse or partner or a person who lives at the same address as you. • Items to note: • Turn around time for your Corporate Card Application is a maximum of 10 working days upon receipt by American Express; missing or inaccurate information may lead to delays. • If you need your card urgently, tick the box on the top portion of your application form & your application will be given priority handling. A fee of $50 priority handling fee will be billed to your Corporate Card Account. • Application status enquiries can be made to Amex Telephone Service Centre at 0800 722333 – Amex will only be able to advise status once the application has been received from your Program Administrator.
What is the certification process? • The applicant must arrange for an acceptable certifier to certify the documents. • He / She will need to show the certifier the original identification documents, plus the copies to be certified. • The certifier will need to certify these copies as “True Copies” of the original document. • To certify the documents, the certifier should use the wording below, the wording needs to be written on the same page as • The photocopied document, if the certification is on a separate page the copy will need to be certified: • I certify this to be a true copy and represents the identity of Mrs XYZ, DL#123“ • Full name of certifier_____________________ • Signature of the certifier ___________________ • Date that the document was certified__________ • Category / qualification of certifier (for example, JP, Accountant etc) ____________ • and Stamp, seal or identification number of certifier.
