DEFINITION • It is the external application of water for ttt purposes.

1. Thermal properties: • Water can be applied hot, cold or neutral in temperature. • The same physiological effect of heat and cold are applied to hydrotherapy as to other thermal agents except that larger body surface area is usually immersed in water than that covered by a hot or cold pack. • Therefore, hydrotherapy has both local and general effects on the body. • The greater the difference between skin and water the more intense the reaction.
2. Mechanical effects: a) buoyancy: • it can be used to assists with exercise of the extremities and to minimize stress on joints and ms. • Work can be done in the opposite direction of buoyancy so that the resistance of motion can be exerted. b) Cohesion and viscosity; • Water has more cohesion than alcohol and therefore provides more resistance to objects moving through it. c) Tactile or pressure: • Water exerts a perpendicular pressure against the surface of the body. • The impact of water under pressure is used to clean wound, influence sensory nerves and influence peripheral circulation.
3. Chemical effects: - According to the content of water, it may be a medium or a method of carrying cleaning or medicating agents to a large area of the skin.
Hydrotherapy forms • Immersion (locally or generally). • Pouring liquid over the part or by douches or shower. • Compresses or packs and massage (ice).
Hydrotherapy with mechanical stimulation Whirlpool bath: • A local bath for extremities with the limb submerges in rapidly circulating water. • Of any size, tub or tank for the whole body immersion.
Therapeutic effects • Effects: • Conductive heat + gentle massage which: • 1. Has sedative effect, • 2. Relieves pain, • 3. Relaxes ms • By: • Vasodilatation • Improves local arterial and lymphatic circulation. • Softens scar tissue. • Breaks down old adhesions after fractures. • It cleans and stimulates wounds. • Mechanically removes dirty , dead tissue and pus. • Regress inflammatory processes. • Diminution of edema and effusion.
Types of whirlpool bath • Combination arm/leg and hip whirlpool tank. • Vibrabath unit • Jaccuzi (hydromassage unit) Temperature: 36- 41 C. Duration: 15- 45 min.
Indications: • Poor circulation • Chronic edema • Separate dead tissue • Pain relief as in phantom pain • After cast in fractures • Stiff joints • Infected wounds • Arthritis • Poliomyelitis and paraplegia • Traumatic and chronic inflammatory conditions • Preparatory to massage, passive stretching and exercises.
Contraindications • Diabetes • Varicose veins • Advanced atherosclerosis • Advanced peripheral vascular diseases
Hubbard tanks • It is a large bath used for total body immersion. • Made of stainless steel or plastic. • Looks like figure 8 allowing the physical therapist to treat without entering the water. • Dimensions are 2 meters long, 1.5 meter wide and 60 cm deep. • A water stretcher and overhead carrier may be used for non-ambulatory patients in and out of the tank. • One or more turbines used for agitation of water.
Therapeutic effects • The same as whirlpool bath. • temperature: 36- 41 C. • Duration: 15- 30 min.
Indications and contraindications • The same as whirlpool bath
Contrast bath • The alternate immersion of the part in hot and cold water. • Two baths are used, one with hot water (40- 45 C) and the other with cold water (15 C). • ttt should begin with and end with hot water but some therapist prefer to end with cold water. • Place the limb in hot water for 3 min then immediately in cold water for 1 min. The whole process should be not more than 15 min.
Physiological effects • Marked and immediate vasodilatation. • Rapid increase of the skin temperature. • Reflex increase of deeper circulation. • Marked sedative effect.
Indications • Posttraumatic swelling • Painful swellings • Chronic inflammation Contraindications • Advanced peripheral vascular diseases. • Arterial insufficiency • Diabetes
