Improving Outcomes Guidance • NICE CYPIOG August 2005 > an IOG defined by age • Improve clinical outcomes and experience of young people AND their families • Safe and effective services as locally as possible, NOT local services as safely as possible • PTCs providing sustainable services with clearly defined staffing • Shared care with defined agreed responsibility • Care delivered by tumour specific Multidisciplinary Teams (MDTs) • Skilled, professional key workers identified to coordinate care; provide information; assess needs • < 19yrs at PTC and > 19yrs to have unhindered access • Offered clinical trials, treatment and care via agreed protocols and pathways • Registration of cancer in 15 – 24yr olds • Implementation by December 2010 South West Specialised Commissioning Group

  4. What has been achieved so far? (1) • Stakeholder event > Implementation Summary 30th September 2008 to NCAT with proposed model of PTC in Bristol and up to 8 Shared Care Centres > HOSC briefing and agreement • Project Implementation Plan > reviewed quarterly • Cancer Networks > Children and Young People’s Site Specific Group which includes TYA • A UH Bristol TYA Advisory Group and more recently a TYA Project Steering Group which report to the Trust Executive > PTC plans and business case development • Appointment of a TYA MDT Co-ordinator at UH Bristol (Trust funded) • Appointment of a TYA Lead Nurse in UH Bristol (TCT funded) • Appointment of a p/t TYA Clinical Lead at UH Bristol (Trust funded) South West Specialised Commissioning Group

  5. What has been achieved so far? (2) • Investment for 09/10 onwards to appoint up to four f/t Shared Care Clinical Liaison posts across the South West (PCT funded) • Commissioner investment towards sessions for Psychological Support for TYA (PCT funded) • A TYA MDaT to support the treatment of patients across the South West, including a pilot collaboration with external partners to enable a secure virtual platform via which to hold the MDaT • Identified site specific ‘tumour champions’ UH Bristol > TYA MDaT • £15k non-clinical research grant to research and audit the benefit of the virtual online platform in facilitating teams; building relationships and improving patient outcomes > future PCT investment • Funding for developing systems at PTC to report all TYA with cancer, 15 to 24 years > 1st January 2009 (TYAC funded) • Teenage Cancer Trust South West fundraisingcampaign launched South West Specialised Commissioning Group

  6. What has been achieved so far? (3) • UH Bristol Oncology Unit ‘Refresh Project’ - enhancements of identified facilities for TYAs (TCT funded) • Computer internet access and printer in the Cancer Support Centre at UH Bristol for patients and their families > internet access for all TYA with cancer when receiving treatment in hospital (TCT funded) • A South West wide User Involvement Group (UIG) of cancer network reps > development of a web site to support patients and families in developing a virtual network which can engage and involve TYA (Cancer Network funded) • Data Group to look at patient flows and pathways > UH Bristol • Development and circulation of Shared Care proforma > up to 6 possible Shared Care Centres across South West agreed • SW SCG taking lead on CTYA Supranetwork Group > PTC and shared care mechanism for sharing practice and expertise • National project in progress around supporting patient choice South West Specialised Commissioning Group

Next Steps (1) • PTC and Shared Care partnership planning event > sharing plans and identifying common issues > agreeing way forward • Scoping and planning for future designation of Shared Care Centres with PTC • Workforce gap analysis and planning > future investment • Discussions and appointment of Clinical Liaison posts • Roll out of virtual online platform across South West > evaluation and audit > Staff training • Further development of TYA MDaT SOPS in liaison with Shared Care Centres > TYA MDaT to include ALL South West patients South West Specialised Commissioning Group

Next Steps (2) • Development of clearly defined patient pathways via PTC and Shared Care Centres > Choice project • Identify ‘TYA Champions’ for each tumour site across South West > network of clinical expertise • Further development of South West wide web site • Development / Appointment of Family Support Network and Activity Co-ordinator posts South West Specialised Commissioning Group
