  2. Crisis in Martin County • In January 2011, 85% of TD funding was consumed by dialysis members • Only 15% was available to the remaining TD population • TD members complained to LCB-TD members, Commissioners, MPO, and MTM about lack of transportation

  3. TD Steering Committee Members: MTM, FDOT,MPO and SFCS • The committee developed a list of alternatives to increase capacity: • Partner with nursing homeswith available transportation • Transition dialysismembers to public transit • Work with dialysis center • Implement a dialysis carpool program • Reach out to family members • None of these initiatives were successful

Action • Removed several dialysis members from transportation on July 1, 2011 • The LCB-TD ruled that only 50% of TD funding can be used for recurring or dialysis transportation • Nine dialysis members were removed from transportation and placed on a “Wait List” • This action sent a strong message to the community that they needed to partner with us and become part of the solution

  5. Newly Developed Partnership • Nursing Homes • Provides transportation twice weekly for their residents • Removes 64 trips monthly from the TD system • Working to increase transportation budget which will allow for a full-time driver • Working on purchasing a wheelchair accessible vehicle

  6. Newly Developed Partnership • Dialysis Center • Worked with MTM on changing member chair times which resulted in the creation of four routes • Hobe Sound to Stuart • Palm City to Stuart • Indiantown to Stuart • Stuart to Stuart

Newly Developed Partnership • Dialysis Center (Continued) • Renegotiated rates offering transportation providers a multi-loaded/group rate • Reduced transportation costs by $3.00/per trip • Transportation is coordinated by the PM and social worker, then provided to the member • Fresenius provides $50/annually to each dialysis member - $1700 or 425 Trips

  8. Newly Developed Partnership • Family Members • Transport members whenever possible • MTM worked with local cab company on fair rate for those that are able to pay • Only call fortransportation whenabsolutely necessary • Vacation • Vehicle break-down • Work schedule
