Health Care Reform – True or False?
Cabin Creek Health Systems • An FQHC with 4 centers in Kanawha County, WV • 2,500 patients 65 and over • 100 staff members • Implemented Senior Medical Home in 2008 – funded by the Claude Worthington Benedum Foundation • Medical Homes = really good primary care
Why Focus on Elderly Patients? • Multiple chronic conditions • Big variation in home/family support • Social issues aren’t being addressed • Many, many medicines – opportunities for harm • Fat complicated medical records – error prone • Multiple medical providers • Lack of coordination between hospital/ER and primary care providers
Senior Medical Home More than a provider of medications and procedures……….. REQUIRES CARE COORDINATION
Senior Medical Home – Numbers • Currently 630 patients enrolled • Avg age 82, 74% female • 87% Diabetic • 90% Hypertension • 68% Heart Disease • 42% Chronic Pain • 56% Psychiatric Diagnosis • 263 home visits • 3,000 care coordinator contacts • 6 ramps built, 3 ready for installation • 84 grab bars installed • 30 trips to visit patients in the hospital • Linking patients with outside resources • Educating all about health care reform
Health Care Law Health care reform bill passed by congress on 3/23/2010
Medicare Changes • All of the traditional Medicare benefits are maintained • Closes the doughnut hole in Medicare Part D, completely closed by 2020 • Improved preventive benefits • No cost sharing for US Preventive Serves Task Force recommended services • Free annual wellness visits, including health risk assessment and personalized prevention plan
Senior Medical Home and Reform • Center for Medicare and Medicaid Innovation (CMI) to pilot Advance Primary Care Demonstration Project • Primary care centers are paid a monthly care management fee for each Medicare patient enrolled Reducing costs and improving quality!!
