  3. Road fatalities in 2009وفيات الطرق لعام LTI No 4 2009 6th February What Happened? A water tanker was traveling on the graded road from Yibal, heading towards the Lekhwair-Fahud black top road when it struck the side of a pick up truck carrying 3 employees. The pick up truck was turning into the graded road from a newly constructed junction. • Result • Death of pickup driver • Two passengers minor injuries • Life saving rules • While driving, no one shall use mobile phones, including hands-free/blue tooth GSM or two-way radio. • Drivers shall never exceed posted speed limits. • Drivers shall never overtake in a dust cloud nor tamper with a vehicle’s safety devices. • Lateral learning • Failure to stop at the STOP sign at the junction. • Defensive drivers act so as to always be in control of the situation. • Stop and wait if not sure

Driver and passenger had fallen asleep at the wheel. RESULT Two third party fatalities and 2 Weatheford LTIs • LATERAL LEARNING • Ensuring fitness for work, in particularly during Ramadan

  7. Life-Saving Rules Follow the prescribed Safe Journey Management Plan. Obtain authorization or a valid Permit to Work before overriding or disabling safety-critical equipment. Verify isolation before work begins and use the specified life-protecting equipment. While driving, do not use a mobile phone and do not exceed speed limits. Obtain a valid Permit to Work whenever required for the task at hand. Do not walk under a suspended load. 1 3 5 7 9 11 2 4 6 8 10 12 Conduct gas tests whenever required. Obtain authorization or a valid Permit to Work before entering a confined space. Use specified fall-prevention equipment when working at height. Do not light up cigarettes, cigars or pipes in no-smoking areas. Wear seat belts when in a moving vehicle. Do not drive or work under the effect of drugs or alcohol.
