Introduction : • Introduce yourself… • Allowance must be made for prematurity ( for interpretation of growth & Dev.) • Review of Information (Antenatal, SCBU, Discharge letters….) • F.H & Social history : Correct any error.
History : • Any worries about the baby ? • Any problem with feeding ? Breast / Bottle • Is the baby smiling ? • Does he look at mother & watch where she is going ? • Does he respond to her voice ?
Observation : • Is the baby alert & interested in his surroundings ? • Does the baby dislike being handled ? • Is he adequately clean & clothed ? • What is the quality of the mother’s interaction with her baby ? warm / detached / depressed….
Growth: • Plot weight, OFC, ? Ht. • Centile Chart. • Wt. Loss mostly due to inadequate intake.
COLOUR: • Look for cyanosis…. • Look for jaundice…
SKIN : • General inspection for dermatitis, Icthyosis, monilia,…. • Strawberry Naevi… • Mongolian blue spots (do not confuse with bruises)
HEAD : • Head circumference • Head shape / asymmetry • Cephalhaematoma • Fused I.C sutures ? Craniostenosis • Fontanelle
Dysmorphic features: • Multiple features may point towards specific Syndrome….
Mouth: • Check for cleft palate (can be missed !) • Oral thrush / monilia
Eyes: • Red reflex ( for cataract ) • Look for nystagmus, squint, structural abnormality • Conjunctivitis • Blocked naso-lachrymal duct
EARS: • Malformation (can be associated with Deafness). • Neonatal hearing test.
C.V.S • Look for central cyanosis • Tachypnoea • Femoral pulses • Heart murmur
Respiratory system : • Look for tachypnoea • Look for recessions
ABDOMEN: • ? Abdominal distension • Palpate for liver, spleen, kidneys • Umbilical hernia • Inguinal hernia (Needs referral) • Single umbilical artery
Genitalia • Undescended testicle (F.U) • Hypo & epispadias (Refer) • Hydrocoele (common & generally resolve) • Ambiguous genitalia
HIPS • Will be discussed in detail. • Ultrasound Scan (High risk)
DEVELOPMENT • Posture & gross motor • Vision • Hearing • Social behaviour
OUTCOME: • Normal growth & health : congratulate… • Minimal deviation from normal : Explain & review. • Parental concern (Review). • Unequivocal departure from normal : Explain & refer. • If not sure : Observe / Discuss / Refer.
