Prevention Agenda Dashboard • Interactive web-based visual presentation of the tracking indicator data: • 96 indicators and sub-indicators at state level • 68 indicators and sub-indicators at county level • Serves as a major data source for assessing, planning, monitoring and evaluating health interventions and progress made toward meeting: • Prevention Agenda 2017 objectives. • Objectives of health reform efforts including DSRIP and SHIP. • Audience: state and local government agencies, policy and decision makers, non profit and private organizations and communities.
