  1. SWIFT SWIFT Complete the contact choosing the appropriate “A-AP Alert-No Further Action” outcome and populate the outcome date. Text should be added to the “Outcome Text” field to indicate the reason for the decision. Within 24hrs of the contact being raised, the Team Manager must decide whether this AP Alert is an AP referral requiring further investigation Closed AP Contact – qualified by Team Manager as “A-AP Alert-No Further Action AP Team - build a cumulative picture of all AP Alerts launched to Key Teams using the AP Contacts Reports in Infoview. SWIFT Close the corresponding Hazard by specifying a closure date SWIFT Re-launch a job to the AP Team Manager and the Key Worker if appropriate, as an “ AP Concern” within the Action Tab, choosing the appropriate “A- Adult Protection Assessment” Outcome, and accurately populating the outcome date. This will result in the AP Team recording this contact as a new AP Incident on the AP Module. Only Safeguarding AP Referrals to be Launched to AP Team Manager Infoview Report will be generated to show how effectively teams have responded to these alerts within 24hrs. A sample should also be taken by the AP Team to scrutinise the judgements - Overview of all AP Contacts launched available via INFOVIEW Reports 2-5 CADT or EDT launch a workflow job as “Decision Required”, stipulating the date following the contact date in the Action By Date Field, to the appropriate Locality Access Team, NB- Record the contact as an AP Concern and leave the Outcome field blank 6 AP Alert not requiring further investigation Hospital admin launch a workflow job as “Decision Required”, stipulating the date following the contact date in the Action By Date Field, to Hospital Discharge Team, NB- Record the contact as an AP Concern and leave the Outcome field blank AP Alert requiring further investigation 7-10 AP Concern not requiring further investigation SWIFT To be completed within 24 hrs Complete the “decision required” workflow job SWIFT screen shot details are available AP Concern requiring further investigation Details from the Incident Alert form recorded in AP module by DASS SA team Email the Safeguarding incident alert formas a Weblink to the DASS SA Team Complete the relevant Safeguarding incident alert form (ESCR 26) To be completed within 5 Working Days

  2. CONTACT RECEIVING TEAM • 1. Contact Receiving Team (CADT/EDT/Hospital Admin) must enter details of person within SWIFT where person is not known. Date of Birth and ethnic origin must be recorded.

  3. 2. Contact receiving Team (CADT/EDT/Hospital Admin Team) will record details of contact. Source Type and reason of “Adult Protection concern” must be recorded. Outcome must be left blank

  4. 3. Contact Receiving Team (CADT/EDT/Hospital Admin Team) will forward a “Decision Required” workflow job to appropriate Locality Access Team. Action by date will be date following contact date.

  5. 4. Contact receiving Team (CADT/EDT/Hospital Admin Team) will ensure that there is a main category recorded for each person where a safeguarding issue has been raised. Main Category end date must be left blank. Return to Map

  6. B. LOCALITY ACCESS TEAM/ HOSPITAL DISCHARGE TEAM Locality Access Team /Hospital Discharge Team will complete Contact Tab by inserting appropriate outcome and outcome date. Outcome date will be date outcome decision taken. Return to Map

  8. NO CURRENT KEY WORKER 3A. If Contact outcome = “A-Adult Protection Assessment” Locality Access Team/Hospital Discharge Team will forward workflow job to Adult Protection Unit indicating “Adult Protection Concern”. Action by Date will be day following Action date.

  9. CURRENT KEY WORKER 3B. If Contact outcome = “A-Adult Protection Assessment” and there is a current Key Worker, Locality Team/Hospital Discharge Team will forward workflow job to Adult Protection Team. Locality Teams will forward workflow job to relevant locality key worker. Hospital Discharge Team will inform key worker of episode but will co-ordinate assessment with WUTH Safeguarding Team. Action by Date will be day following Action date. Return to Map
