Nursing Practice Models: Implications for Information System Design
What are NPMs? • Describe the structure and contextual features of nursing practice environments. • Made up of 11 factors that provide administrators with specific features of the clinical practice. • The author’s believe these factors are a good guide when designing an Info Systmem to support a clinical practice.
Continuity of Care Participation in Management Meathods of Collaboration Leadership Type Nurse’s Role in Care Staffing Levels Communication Specialization Orientation of Temp Staff Group Commitment What are the 11 NPM Factors?
How can these Factors be used as a guide? • Each factor has an operational definition. • The operational definition provide design criteria. • They make explicit the nature of the information needed and the pattern of communication. • These factors will very from Unit to Unit.
Nursing 18 RN and 22 LPNs for 23 patients using a functional assignment pattern. Head Nurse takes patients. Quality Program monitors for errors, provides feedback. Info System IS records ADT protocols and written care plan. Nurse Specific and Patient Specific data are necessary for timely care. Access to policies are needed. Terminals need to be deploy to maximize space on the unit. Example 1:
Nursing 17 RN and 2 Clin Specs for 23 patients Shared governance committee: nurses, physicians, social workers establish protocols and policies. Staff works in a highly collaborative manner. Quality assurance focuses on process improvement. Info System In addition to ADT this unit requires EMR that covers all aspects of care. Staff would benefit from access to knowledge resources, quality standards, and self learning modules throughout unit. Quality programs need access to NCQA databases to permite rapid adjustment of care processes. Example 2:
Conclusion • Nursing Practice Models provide a guidance, and not guidelines, when designing a clinical info system. • The provide the questions to ask that will lead to a successful system. • The Nursing Practice Models do not provide a complete guide. The needs of ancillary departments and patient populations must also be considered.
