  4. Core Benefit Solutions That Work For You Employer Paid Benefits • Basic Life and AD&D – Employee 1.5 x salary to a maximum of $300,000, Spouse Life Coverage of $5,000 and Dep. Child Life Coverage of $2,500 • Short Term Disability – 60% Pre-Disability Earnings up to $1,000 maximum per Week • Long Term Disability – 60% Pre-Disability Earnings up to $10,000 maximum per Month

  6. ImportantNote! Medical Benefits – United HealthCare While the plan you select may not require you to use a primary care physician, it remains your responsibility to make sure you are using In-Network providers in order to enjoy the benefits of the Plan’s In-Network benefit schedule.

  8. ImportantNote! Dental Benefits Your Dental Plan covers four main types of expenses: This chart shows how much you pay for certain products and services. Keep in mind, if your doctor is out of network and possibly charges more than the Plan’s “reasonable and customary” charge, you may be required to pay the extra amount.

Vision Benefits Your Vision Plan covers the following types of expenses: * If prescribed, one pair of standard single vision or standard multi-focal lenses **The fitting/ evaluation fees, contact lenses, and up to two follow up visits are covered in full (after Copay). If you choose disposable contacts, up to 4 boxes are included when obtained from a Network Provider.

  13. How you can save money • Use your Preventive Care for your Annual Wellness Visit, covered at 100% • Use primary care doctors instead of specialists • Use Urgent Care in place of the Emergency • Use Generic or Mail Order Prescriptions

Life Insurance Benefits • Your Basic Life and AD&D insurance are paid for by BCI Broadband • 1.5 times annual salary to a maximum of $300,000 for Life and AD&D • $5,000 Spouse & $2,500 per Child for Life Only • You can purchase additional Supplemental Life Insurance for you, your spouse, and your children through UNUM • Employee Coverage can be elected up to 5 x annual salary to a maximum of $250,000 • Spouse coverage can be elected up to 50% of the Employee amount to a maximum of $50,000 • Dep. Child coverage can be elected up to $5,000 per eligible dependent child.

Short and Long Term Disability Benefits • Short Term Disability benefit • 60% of Pre-Disability Earnings • $1,000 weekly maximum • Elimination period -30 days for Injury or Illness • Long Term Disability benefit • 60% of Pre-Disability Earnings • $10,000 monthly maximum • Elimination Period - 180 days
