FY 2014 Contract Enhancements Renew Application via Annual Certification ~ Use if there are no or very few changes to contract ~ Link can only be used once per year; additional updates will need to be completed by accessing the second button, “Enter New-Update Application Information”

FY 2014 Contract Reminders • Licensed Agencies • Does your current license data match the information on each Site Application page? • If different, update the Site Application page and submit a copy of the current license to DPI • Agencies submitting Health and Safety Documentation • Information must be submitted to DPI annually • At-Risk Programs • Recommend updating school enrollment data annually to re-establish the 5 year approval window • Use School Fiscal Year 2013 data
FY 2014 Contract Reminders • Revise pages where the agency has entered data with the year specified (i.e. 2013) to list only months; do not specify a year • Agencies with a Board of Directors • Board Information Page - months of board meetings • Sponsoring Organizations • Training Topics Page – months of training • Site Meals Information Page - months of monitoring reviews
FY 2014 Contract Enhancements • Board Member or Corporate Official/Board Information • If your agency’s President, Vice President, Secretary or Treasurer have changed they need to be updated in contract • Answers on the second board of director’s information page will be cleared and you will have to answer the questions again relevant to the new information
Board of Director Rules Reminders • The Executive Director or Authorized Representative can be a member of the board provided a resolution is in place • Resolution – bylaws, articles of incorporation or board policies and procedures include a resolution that the Executive Director or Authorized Representative is not eligible to vote on items related to the board’s decision regarding their salary or other human resource issues that affect them, such as hiring and firing.
Board of Director Rules Reminders • A majority, 50% and greater, of board members cannot be employed by the agency • Board members cannot be listed in the contract on the Staffing Personnel page as performing CACFP tasks
Board of Director Rules Reminders • Financial Interest • A majority (>50%) of board members cannot have a financial interest in the agency • Financial interest is anything of monetary value, i.e. salary, consulting fees, honoraria, equity interests, interests in real or personal property, rent, debt forgiveness, etc.
Board of Director Rules Reminders • Family Related • Board members cannot be family related to each other • The Executive Director or Authorized Representative cannot be family related to a board member • Board members cannot be family related to personnel performing CACFP tasks as listed on the contract Staffing Personnel page • Family related is defined as an individuals’ spouse, domestic partner or similar designation, and the individual/spouse/domestic partner’s children, grandchildren, great grandchildren, siblings (whole or half blood), and spouses/partners of the children, grandchildren, great grandchildren and siblings
Contract Changes • Staffing Personnel Page • “Maintains participant(s) intake forms/enrollment forms” for the CACFP • "At Risk" After School Programs, Emergency Shelters or Outside School Hours Care Centers no longer have to include job duty because enrollment form (PI-6077) is not required of these types of agencies
