Lambeth Addictions Consortium Anthony Docherty RMN – Deputy Borough Lead/Shared Care Nurse Manager, SLaM Lambeth Addictions Consortium and the Shared Care Team Several years ago Drug and Alcohol treatment providers across Lambeth consisted mainly of; SLaM NHS Foundation Trust Blenheim CDP Foundation 66 & Addaction/Mainliners 3 years ago, along with Lambeth PCT we all agreed to plan and develop a partnership model, this developed into a fully integrated Consortium Treatment Model Mainliners/Addaction
Lambeth Addictions Consortium The Lambeth Treatment Consortium is made up of 3 main Teams The Opiate Pathway Team Alcohol and other drugs (stimulant, cannabis etc) Pathway Team And The Shared Care Team (SCT) (each team is made up of all 4 providers) -Also SMT partners and ACTS- There are 3 main sites across Lambeth; Lorraine Hewitt House – Brixton The Harbour – Loughborough Junction Addaction/Mainliners – in Streatham Lambeth residents can attend any of the services for an assessment including their GP and be offered the same treatment package no matter where they attend Mainliners/Addaction
Lambeth Addictions Consortium • History of Shared Care provision in Primary Care: CLAS model and Dr Clare Gerada, Stockwell etc – • The Lambeth Shared Care Team comprises of 14 keyworking staff – nurses, drug and alcohol workers and alcohol brief treatment staff, psychology lead and our SMT and CYPS colleagues • Joint SLaM/Kings Hepatitis nurse: in-house Hep’ Tx • & myself as Lead Clinical Nurse Manager and • Dr Mike Kelleher, Addictions Consultant Psychiatrist • Weekly MDT to discuss SCT • Lambeth Clients currently active in Treatment (d&a) = 1400+ • Clients currently seen in shared care = 500+ • In Lambeth there are 50+ GP Practices • We hold weekly Shared Care clinics in 37 surgeries • With a Total of 53 GP shared Care clinics across Lambeth Mainliners/Addaction
Lambeth Addictions Consortium Why Shared Care in Lambeth works Clients can be book in to see a SCT worker themselves If Opiate problem, then same day prescribing/titration happens within primary care Clients are seen weekly or fortnightly or offered extra time at other location ALL clients have their ‘own’ Addictions Recovery Care Plan (ARC) Clinical notes, copies of care plans, risk assessments etc are all entered onto the surgery Emis system GP’s have a role in ARC and clients physical h/care needs met Weekly/Monthly Review meetings with GP and worker Drug/Alcohol detox and rehab can be arranged by their SCT worker 12 Lambeth pharmacists trained to carry out Alcohol Detox for shared care clients Excellent links with ‘talking therapies’ - IAPTS Mainliners/Addaction
Lambeth Addictions Consortium Why Shared Care in Lambeth works PCSMAG and GP Forums held GP’s can call myself/Mike anytime to troubleshoot We also aim to carry out yearly clinic reviews with our GP partners If required, smooth transfer of clients back into LHH LHH/LH transfers out to primary care; adequate planning, 3 way meetings, meeting with GP, 3 month trial etc Lambeth Shared Care SU’s monthly meeting held Yearly SU Audits and PEDIC carried out – action plans developed All shared care clients are referred into any Consortium Groups across the Borough – SCT have the highest referrals into the group programme SC clients also attend the Recovery Space: ACTS Benefits, housing workers, complimentary therapies, access to further education etc Creche’ and Parenting Skills Groups Aurora Peer Mentorship Service Mainliners/Addaction

Lambeth Addictions Consortium - To summarise - All our clients receive Recovery-based Care Plans and are fully engaged with these, starting from ‘what Recovery means to them’ They have access to all recovery focussed groups across the Borough We have very experienced GP’s that are addictions-trained and interested in working with our client group & in turn our clients genuinely feel ‘cared for and well looked after’ Our SC model is tried and tested, we do have ongoing excellent SU input and we do make changes and adapt when our SU’s give us feedback, they are our key stakeholders to our model being successful THANK YOU Anthony Docherty, Slam NHS Foundation Trust.
