CONGENITAL SYPHILIS METAPHYSITIS : • Transverse radiolucent band • Serrated metaphyseal ends = fraying • Bone destruction at metaphyseal margin adjacent to epiphysis • Wimberger’s sign = bone destruction at medial upper tibia DIAPHYSITIS : • Scattered bone destruction +/- expansion • Solid or lamellated periosteal reaction

  4. RICKETS ( Vitamin D deficiency ) Increased uncalcified osteoid in immature skeleton X-RAY FINDINGS : Epiphyseal change : • Widening of epiphyseal cartilaginous plate • Loss of epiphyseal definition

  5. RICKETS Metaphyseal change : • Loss of zone of provisional calcification • Metaphyseal fraying = irregular metaphyseal end • Metaphyseal cupping • Metaphyseal flaring • Thin bony spur • Indistinct cortex ( uncalcified subperiosteal osteoid )

  6. Widening of epi. cartilagenous plate Loss of zone ofprovisional calcification Metaphyseal cupping, flaring and fraying Indistinct cortex and loss of epiphyseal definition

  7. SCURVY Deficiency of Vitamin C Onset : 6 months - 2 years , rare in adult S/S : Leg/hip/knee pain , refuse to walk X-RAY FINDINGS : • Osteoporosis • Thinning of cortex • Thin, dense ring like calcification around epiphysis “ Wimberger’s ring “
