Indications and Usage The recommended dosage for previously untreated adults (normal renal function) is 400mg/m² as a single intravenous infusion over 15-60 minutes. Therapy should not be repeated again until four weeks have elapsed. In patients with risk factors such as previous myelosuppressive therapy or in the aged, the initial dosage may need to be reduced to 20-25%. Patients with Impaired Kidney Function Patients with creatinine clearance values below 60 mL/min are at increased risk of severe bone marrow suppression. In renally-impaired patients who received single-agent carboplatin therapy, the incidence of severeleukopenia, neutropenia, or thrombocytopenia has been about 25% when the dosage modifications in the table below have been used. Side Effects Relative to cisplatin, the greatest benefit of carboplatin is its reduced side effects, particularly the elimination of nephrotoxic Nauseaand vomiting are less severe and more easily controlled. The main drawback of carboplatin is its myelosuppressive effect. This causes the bloodcell and platelet output of bone marrow in the body to decrease quite dramatically, sometimes as low as 10% of its usual production levels. Mechanism of Action Carboplatin is a cisplatin derivative and shares the same mechanism of action. Highly reactive latinum complexes are formed intracellularly. These complexes inhibit DNA synthesis through covalent binding of DNA molecules to form intrastrand and interstrand DNA crosslinks.
