Synchronization of Medications • Works similar to setting up a medical appointment. • All medications will be filled once on a monthly basis or 3 month basis. • Pharmacy communicates with Patients one week prior to refilling medications. • Pharmacy staff prepares medications. • Patients pick up all refill medications on an assigned date.

  3. Medication Therapy Management (MTM) • Pharmacy will set up an appointment for patients to meet with the pharmacist. • Prior to meeting, the pharmacist reviews records and refills of a patients adherence to taking medications on a regular basis. • During the meeting the pharmacist will listen to concerns a patient has about their drug therapy.

  4. Medication Therapy Management (MTM) • Discuss side effects • Drug interactions • Review Vitamins and Supplements, including Herbal supplements • Review Over the Counter Drugs (OTCs) • Talk about adherence problems

Immunizations • All immunizations are reported to a central state data base for medical and pharmacy access. • Flu vaccine ( quadravalent and HD for 65 years and older) • Shingles Vaccine (Shingrix: two shot series) • Pneumonia Vaccine: Prevnar: two shot series • Tetnus, Diptheria and Pertussis: Tdap

  6. Nutritional Supplements • B-Complex Vitamin (methylated B vitamins) • Vitamin D 2,000 Units to 10,000 Units daily. 50,000 Units weekly. Blood Level: 30 to 70. • Fish Oils 1125 EPA/ 875 DHA (Fish Oil should not smell fish • CoQ10 100mg (Ubiquinol) • Magnesium Glycinate 200 to 500mg • Probiotics

  7. Nutritional Supplements • Vitamin C • Vitamin A and E • Minerals: Zinc, Selenium, Chromium • Digestive Enzyme • Liver Detox supplements • Protein Powders • Alpha Lipoic Acid • Hair, Skin and Nails

Diseases to consider when taking Over the Counter Drugs (OTCs) • Asthma • Bleeding disorders • Clotting disorders • Diabetes • Enlarged prostate gland • Epilepsy • Glaucoma • Heart disease • High blood pressure • Immune system problems • Kidney problems • Liver problems • Parkinson’s disease • Psychiatric problems • Thyroid disease

OTC Drugs • Pain relievers: Aspirin, Ibuprofen, Naproxen and Acetaminophen. • Decongestants: Pseudoephedrine (Sudafed) • Antihistamines: Diphenhydramine (Benadryl) Chlorpheniramine, Certrizine (Zyrtec), fexofenadine (Allegra), Loratadine (Clariten) • Acid Blockers: Ranitidine (Zantac), Famotidine (Pepcid), Omeprazole (Prilosec), Esomeprazole (Nexium) (PPIs)

  10. OTC Drugs • Decongestant Sprays: Phenylephrine, Oxymetazoline, Saline Nasal Sprays • Steroid Nasal Sprays: Nasacort, Rhinocort, (Fluticasone) Flonase • Laxatives and Stool softners • Anti-diarrials: Loperamide (Imodium) and PeptoBismol • Probiotics (Floragen and VSL)
