4 Major Objectives • improve the self efficacy of patients and Canadians on the prevention and control of hypertension; • improve the management of hypertension by health care professionals; • prevent hypertension by reducing dietary sodium; • develop a national hypertension surveillance system to document the above and allow continuous quality improvement.
Partners • Approximately 150 health care professional volunteers with a common vision; • Many national organizations involved - Blood Pressure Canada, The Canadian Hypertension Society, the Heart and Stroke Foundation of Canada, Canadian Cardiovascular Society, Canadian Stroke Network, the Canadian Society of Nephrology, the Canadian Council of Cardiovascular Nurses and Dietitians of Canada, Canadian Pharmacists Association, College of Family Physicians of Canada; • Government - Public Health Agency of Canada, Health Canada, Statistics Canada, some Provinces; • Pharmaceutical and device industry; • Food Industry;
