Grade PS Program Service Disability Code: • Preschool-aged student with a Disability – 5786. Use this code ONLY if the child was classified by CPSE). If the student is in the determination process (4034/140) do NOT report this disability type until the child is classified by CPSE and SPED services start.
Other “PreK” Program Fact (Services) Codes: • “Other” PreK program — 990 Level Designation: District-level service. Description: Indicates participation in a prekindergarten program other than UPK. Entry Date: Date student enrolls in the program. Exit Date: Date the student completes the program or date the student terminates the program without completing it. Reason for Ending Code: Not used • Preschool-aged student with a Disability – 5786 (ONLY if the child was classified by CPSE)
UPK Program Fact (Services) Codes: • “Universal” PreK program —902 (Students reported with a 902 Program Service Code must also be reported with a Universal PreK Setting code (see below) Level Designation: District-level service. Description: Indicates participation in a Universal Prekindergarten Program. Entry Date: Date student enrolls in the program. Exit Date: Date the student completes the program or date the student terminates the program without completing it. Reason for Ending Code: Not used • Type: Universal PreK Setting Codes (Report one ONLY if the student was reported with a 902 code above) 1309 District-operated; 1320 Day care center; 1331 Head Start; 1342 Family or Group Day Care; 1353 Nursery School; 1364 BOCES; 1375 Special Ed 4410 Preschool; 1386 Nonpublic School; 1397 Museum; 1408 Library; 1419 Other • Preschool-aged student with a Disability – 5786 (ONLY if the child was classified by CPSE)
Grade “PS” Review: • Grade PS Student: • You are reporting their services received only. Not their enrollment in a building. • The BEDS of location is their SPED service provider code. • They are the only students to get the codes (4034 and 140 – determination process codes) • The 4034 and 140 codes are the only enrollment codes that can have the district BEDS of location (District code +0000) in the enrollment screen. • 4034 is ALWAYS ended with a 140 code. A 140 code ends the determination process for any reason (i.e. parents stop evaluation or a meeting was held an a decision was made) • The 5786 program service code (preschool aged student with a disability) is only assigned if the child is found to be classified (following 4034/140 codes) and services start. • If a child received summer CPSE services as a grade PS student – they need the appropriate enrollment records and program service records.
Grade “PKH” or “PKF” Review: • Grade PKH or PKF Student: • You are reporting their enrollment in a building/program (UPK or Prek Program) • Report EVERY Grade PKH or PKF students (classified or not) • There are two types of PreK programs: UPK and “other” Prek • If the program is a DISTRICT-OPERATED UPK program, you need: Your district building BEDS code as the location code in enrollment and Grade PKH or PKF. You also need TWO program service codes: • 902 Universal PreK Program • 1309 District-operated setting type • If the program is a UPK Program NOT district-operated, you need: Your district code +0666 at the end as the location code in enrollment and Grade PKH or PKF. You also need TWO Program Service Codes: • 902 Universal PreK Program • A setting type code – do NOT use 1309 district-operated • If the program is an “Other” PreK program (i.e. BOCES program, not a Universal PreK program), you need: The program BEDS Code (i.e. BOCES code) as the location code in enrollment and Grade PKH or PKF. You also need ONE program service code: • 990 “Other” PreK Program
