  1. 18 month enhanced well baby visit audit

Who: Caroline Family Health Team • Where: Burlington, Ontario (catchment area: Hamilton, Niagara, Haldimand, Brant) • When: April 01, 2011 --- September 30, 2011 (six month capture) • What: Chart audit of all eligible children within the practice that would have been 18 months during 6 month time frame

  3. Why: • Is the enhanced visit being performed • Who was completing these visits (GP’s, NP’s) • What percentage of children missed there visit • Was the Rourke being completed • What, if any problem or concern was reported • Was the child referred (Peds, DEIPP, Erin Oak) • Was the child seen in follow up by GP • Was the child immunized

  4. System queried (Practice Solutions)and a list of 88 eligible children was returned

  5. How many of those were a Rourke reported visit 99

  6. How many of those were reported to be an Enhanced visit

  7. Was the child immunized 5% was deferred due to illness

What did we do with this information • We decided to send letter on behalf of the family physician encouraging parents to book an appointment with their health care provider • Developed a pilot “piggy back” appointment. GP’s will visit with the child after there appointment with the NP • Developed an 18 month well baby package to be given out at 15 months. Explaining the importance and difference of the 18 month enhanced visit • Physicians making sure that the health care provider who completed the exam reports it on the Rourke
