  4. Oops! • Paperwork arrives too close to anticipated start date. Sometimes even the day before! Our Vision – Healthy Kansans living in safe and sustainable environments

  5. Yes! • Course Information sheets arrive at least 7 days before course start date. • Applications for Approval of Aide Training Courses arrive at least 3 weeks prior to course date. • Applications with a proposal for pilot online or distance learning technology courses are in the office 4 weeks prior to course date. • Applications with a Request for Waiver of NATCEP Ban arrive 4 weeks prior to course date. • If additional time is required for advertising, the application is submitted early enough to anticipate that need.   6. Oops! • Applications are incomplete. • Signatures are missing. • Class Schedule and Content forms are incorrect. Our Vision – Healthy Kansans living in safe and sustainable environments

  7. Yes! • Sign your documents. • Take time to be sure all forms are correct and complete. • Coordinators share equal responsibility with instructors. Our Vision – Healthy Kansans living in safe and sustainable environments

A special note about Course Schedule and Content Forms • These must be completed by the primary instructor and reviewed by the coordinator. • Are all topics covered? • Are the required number of class, lab and clinical hours documented? • Are breaks and lunch scheduled in addition to course time?   9. Class Schedule and Content Form should be retained by course coordinator and instructor . These only need to be sent in if they are for a pilot course or are specifically requested by Health Occupations Credentialing (HOC).   11. Yes! • Choose either fax, mail or email. Feel free to call and check to be sure it was received. • If emailing applications as attachments, copy Mary Flin in case Dolores is out of the office. Our Vision – Healthy Kansans living in safe and sustainable environments

  12. Oops! • Someone fills out the paperwork for an online or Distance Learning Technology course other than the sponsoring coordinator or primary instructor. Our Vision – Healthy Kansans living in safe and sustainable environments

  13. Yes! • If you are hosting a course, be sure you understand your responsibility and pay close attention to the attestation required. • If the instructor completes paperwork, this instructor must be an employee of the Sponsor.   14. Oops! • Evaluations for waiver, online or DLT courses are not sent in. Our Vision – Healthy Kansans living in safe and sustainable environments

  15. Yes! • Waiver courses, and pilot courses must send all of the required evaluations from students, instructors and coordinators. Our Vision – Healthy Kansans living in safe and sustainable environments

  16. Oops! • The Instruction Form for the Request for Waiver application is sent in, not the actual Waiver application. Our Vision – Healthy Kansans living in safe and sustainable environments

  17. Yes! • The Instruction Form is there to help you. Don’t send it in. Only send in the Waiver application. • Section A is to be completed and signed by the Sponsor coordinator. • Section B is to be completed and signed by the facility administrator.   18. Oops! • Changes are made to approved courses without KDHE/HOC approval. Our Vision – Healthy Kansans living in safe and sustainable environments

  19. Yes! • Request changes in writing. Email is best. • This includes changes, additions, or deletions of instructors, dates, or training sites, and cancellation of courses. Our Vision – Healthy Kansans living in safe and sustainable environments

  20. Include course number with all correspondence. • Send requests to Dolores Staab. See contact information at conclusion of presentation. Our Vision – Healthy Kansans living in safe and sustainable environments

  21. Oops! • Rosters and fees are not received in a timely manner. Our Vision – Healthy Kansans living in safe and sustainable environments
