Preparing for Cohort Review • Cohort preparation begins when cases are first reported • Preparation continues with regular case management • Case managers complete the demographic information on cohort forms • The remaining information is added as case information becomes known: • During patient interview • Through home assessment • Chart review • Clinic visits • Contact elicitation and investigation • Laboratory reports

Standard Forms • Assist case management • Ensure concise case presentation • Collect essential patient and contact information • Assist in evaluation of key indicators New York City cohort presentation forms include: • Pulmonary or Laryngeal TB Case • Clinically Confirmed or Extrapulmonary TB Case

  4. NYC Presentation Form I : Pulmonary or Laryngeal TB To document cases that require a contact investigation: 2a) sputum smear+, pulmonary 2b) sputum smear -, sputum culture + 2c) pediatric; other resp smear and/or culture+; cavitary, culture -

  5. NYC Presentation Form II : Clinically confirmed or Extrapulmonary TB To document cases that do not require a contact investigation 2a) sputum smear+, pulmonary 2b) sputum smear -, sputum culture + 2c) pediatric; other resp smear and/or culture+; cavitary, culture -
