What is Case Management? • Case Management may be defined as a student-centered, goal-oriented process for assessing the need of an individual for particular services and obtaining those services; it is an approach that seeks to make service delivery to its students: • Integrated Student-Centered • Coordinated Goal-Oriented • Accountable Flexible • Prioritized Cost-Effective • Sustained Comprehensive • *Brandeis University Case Management Study in Partnership with the Center for Human Resources

Benefits of Case Management • Case Management offers schools an opportunity to link with other resources in a coordinated fashion that ultimately benefits the student because it: • Incorporates a coherent, prioritized, multidimensional, problem-solving approach; • Locates the student with a particular target group and social framework; • Seeks to stimulate change both within the student and the overall environments of which he or she is a part • * Brandeis University Case Management Study in Partnership with the Center for Human Resources

Effective Case Management • Locate and guide students through a sequence of steps which usually consists of the following components: • Identify and recruit appropriate students • Coordinate meetings with Graduation Team • Intake & assessment / Peach State Pathway • Design an Individual Student Action Plan (ISAP) • Intervene through brokering, advocating and linking services/strategies • Implement and monitor the ISAP • Evaluate the effectiveness • * Brandeis University Case Management Study in Partnership with the Center for Human Resources

Role of the High School Graduation Coach • In partnership with school administrators • faculty, counselors, social workers, support • staff, and parents, the High School Graduation • Coach has been designated to identify and • coordinate services to a target group of • students who are at risk of high school non- • completion.

  9. Steps in Developing a Caseload • School Staff/Faculty Orientation • Distribution of Referral Forms • Collection of Referral Forms • Identify Your Target Group • Meet with Student and Parent (Orientation, Assessments, & Consent Form) • Determine Level of Intervention

School Staff/Faculty Orientation • Set a date and time with your Principal to facilitate a staff orientation: • Develop an agenda • Review agenda with Principal • Share your role, responsibility, purpose • Share need for school support and partnerships • Inform staff of your target group • Describe the method of implementation • Share referral forms and elaborate • Develop and share an example of a case file/folder • Share school goals / expected outcomes

Distribution of Referral Forms • Distribute the Student Referral Forms during a school staff/faculty meeting. • Distribute the Student Referral Forms in each box of the faculty and staff with an instruction memo attached. • Meet with the Counselors from in the feeder middle school and with high school counselors to ask for referrals via the Referral Forms. • Meet with the school system Social Worker(s) to ask for referrals via the Referral Forms. • Make Referral Forms available to students through a display in an area where students gather or outside the counselor’s office. • Make Referral Forms available through the school PTO or Parent Resource Center.

Identify Your Target Group • 9th thru 12th Graders • Failure to Pass Sections of the GHGT • Credit Deficient • Poor Performance on EOCT’s • Poor/Irregular Attendance • Placed by LEA Committee (not promoted) • Scored “Does Not Meet Expectations” on any Section of CRCT • Non-School-Related Challenges

  13. Examples of Non-Academic Challenges • Pregnancy/Teen Parents • Value placed on education • Family issues or lack of family support • Student motivation • Combating destructive attitudes with staff/peers • Discipline infractions • Problematic associations • Health Issues

Collection of Student Data • School Database (AS400, SASI or School Max) • Local Records (Attendance and Behavior Records, Credits Earned, etc.) • State Standardized Test Scores (CRCT, GHSGT, EOCT) • National Standardized Test Scores (SAT, ACT) • Interview with the student and their parent/guardian • 6. Contact with school staff

  15. Parent Orientation • Meet with both the parent and student • Review role as the HSGC • Complete Student /Family Profile Form • Review Peach State Pathways • Complete Individual Student Action Plan • Sign Parent Consent Form (if required) • Give contact information • Set up next meeting

Family Educational Rights and Privacy ActFERPA • FERPA gives parents and students (age of 18 and over) certain rights to educational records • Right to inspect and review students educational records • Rights to correct records if they believed them to be inaccurate • Right to release any student educational records

Determine Service Strategies • Through this process, the HSGC will identify strategies or services that each student requires to become more successful in school: • Tutoring • Support Groups • Mentoring • Counseling • Health Services • Social Skills / Life Skills • Substance Abuse Education • Vocational Exploration • Meeting Basic Needs
