GEMSS Action Group Leadership 2016

This project involved reviewing and updating existing content for various conditions, adding new content, and sharing results to improve information quality and outreach efforts. Recommendations focused on educational materials, medically related content, templates, and transitions. Collaboration among group members was key.

Familiarize & Share: Results • Shared on Rare Disease Day: February 29, 2016 • Early Intervention providers | Leadership community | Condition specific Facebook groups, Preschool contacts through NH Department of Education, Individual providers • In the week following our outreach efforts there were: • 213 new visitors to GEMSS on Rare Disease Day • 19 new GEMSS Facebook likes • 983 new visitors to GEMSS

  10. Recommendations • Improve & maintain quality of information • Focus & expand educational materials • Continue to strengthen & streamline medically related content • Offer easily accessible templates & fact sheets addressing a broad range of issues • Define & Expand Transitions
