Background • Extensive Consultation Undertaken in the development of ‘Healthy Ageing 2020’ • Key themes identified through consultation translated into work plan as per Strategy • Difficulty Accessing Information • Lack of support for Carers • Problems with Home Based Support • Quality concerns with Residential Care • Boundary Issues • Discharge Planning from Acute care • Confusion around PHO’s • Not responsive to minority groups • Managed Bed Policy
National Activity • Home Based Support experimenting with different models • Ministry of Health undertook evaluation • InterRAI piloted around the country • Trials of different funding models began • Capital and Coast DHB lead the way in terms of complete new model • Ministry of Health began policy work to catch up with change • Reporting requirements changed to monitor progress

1. Streamlined Access The vision: Streamlined access and improved integration of community, primary and specialist services through centralized care planning, assessment, and coordination
2. Restorative Home Care The vision: Active case management model based on goal setting with client, reassessment and shorter intervention
3. Packages of Care The vision: Flexible packages of funding based on identified goals and agreed outcomes
4. InterRAI The vision: Standardised single assessment incorporating all aspects of required service delivery. Baseline data to identify trends for service planning and budgeting
