HPE Screening • This process only applies for screening families with children. • Determination of homelessness is done. • Screening should gather basic background information including the core questions. • Identify barriers to housing. • Assign a barrier level to households based on results of barrier identification. • Conduct a quick eligibility screening for special populations (domestic violence, families with children, adults with physical or mental health disability, adult males for SA, persons with HIV/AIDS and veterans). • Refer clients to prevention, rapid re-housing without shelter, shelter with rapid re-housing, transitional housing, and permanent supportive housing.
