INFORMACIÓNNECESARIA PARA LAS FACTURAS Factura con información de RIF y NIT de la contratista, además, subtotal y total que coincidan con el presupuesto y notificación de adjudicación (entregada por el proyecto PTMR). En caso de ser una Pequeña y Mediana Empresa (PyME) debe incluir el monto del IVA. Nombre o razón social: Petróleos de Venezuela S.A. Dirección: Edificio Petróleos de Venezuela, torre Este, Av. Libertador con calle el Empalme, La Campiña, Caracas, Distrito Capital. RIF de PDVSA: J-00095036-9. NIT de PDVSA: 63995509. RAP de PDVSA: código SAP (información suministrada por la contratista). Se debe incluir el número de notificación que aparece en la adjudicación de notificación. Ejemplo: Ref: PTMR-ESTADO-NTXXXX-XX (Las equis corresponden al número suministrado por el proyecto). La factura debe estar debidamente firmada y sellada con número de Cédula de Identidad, nombre y apellidos por un representante de la contratista.
