We will fulfil our role for the communities we serve As a Trust we need to review and assess our services so that we are aligned to deliver against the role we are best placed to deliver in the system. We cannot address the systemic issues we face in isolation and need to work closely with our partners. The objective: Fulfil our role as a provider of timely, accessible care to the Portsmouth and South East Hampshire communities The challenge: The Hampshire population is set to grow and age significantly faster than the national average, placing increased strain on all aspects of the service. Frailty Intervention Team in Emergency Department Frailty Assessment Unit Older Persons Nurse specialists in AMU Dedicated short stay ward for older people Dedicated complex discharge ward for older people End PJ paralysis Discharge Planning assistants with specific focus on older people Dementia case workers targeting patients in urgent care Discharge to assess beds in collaboration with social care Anticipatory care planning for patients nearing end of life How PHT’s services are being improved further to the implementation of the Trust’s new 5 year strategy
