  3. How did Warwickshire Hospitals do? • Acute Liaison Nurses • Questionnaires for patients • Reasonable Adjustments • Local data for health needs • Equality and Diversity, Safeguarding forums • Community care plans

  4. What do we need to do next? • Increase return of questionnaires • Electronic alerts permissions • Improve reporting of fractures, UTI’s and mental health referrals • Continue education and health promotion • Encourage feedback
