WHAT HAS WORKED WELL • Small team including nurses, surgeons, PT/OT that communicated well together before the collaborative
WHAT HAS WORKED WELL • Nurse champion working directly with the team caring for patients. • Doing the spot checks and giving immediate feedback for correction to team members
WHAT HAS WORKED WELL • Added an LPN to team Monday to Friday whose primary purpose is to functionally mobilize patients. Trained by Rehab • Gets late joints up with RN
WHAT HAS NOT WORKED WELL • When key team members are on vacation especially the nurse champion • Post discharge follow up when the patient is from out of town can be poor • Motivation for surgeons to discharge within 4 days has waned if the patient is reluctant since with the increased bed capacity there has been no increase in funding to do more joint surgeries
TOTAL JOINT DISCHARGE QUESTIONNAIRE Purpose: Measure how well the team is preparing pts for discharge mentally and functionally by: • Asking pts to rate their general confidence level on discharge and at home • Asking pts to indicate if they know how to perform 9 functions necessary for a safe discharge and at home
TOTAL JOINT DISCHARGE QUESTIONNAIRE • Pain adequately controlled • Know signs and symptoms of incision infection • Know when to shower and how to dress incision • Knows how to prevent constipation • Can get on/off bed by self • Can get on/of toilet/commode by self • Can wash and dress self with devices or not • Knows how to do exercise program • Can climb stairs if has them
