  4. Organizations within a PHE • Vaccination Clinics (where the vaccine batch is created) • Referring Organizations • Take Reading Organizations

  7. Process for obtaining PVS authorization • Obtain digital certificate via SDN • Install certificate • Access PVS

  8. Obtain Digital Certificate • Requires an administrative password • Password supplied to VDH by PVS • VDH then supplies to users

  9. Install Digital Certificate • Specific to machine • Not transferable* • Avoid laptops, unless printer/high speed access consistently available • Follow instructions as supplied by SDN • Export certificate

  10. Access PVS • Follow detailed instructions to access PVS (to come later) • Requires another password • Password available from VDH

  12. Data Entry Tips • Create batch by choosing lot # of vaccine & lot # of diluent • Print clinic/batch information • Copy • Attach copy to each patient history

  13. Active Surveillance • Link to this web page via PVS • PVS users automatically have access • Complete 21-28 days post-vaccination
