HIGH ALERT MEDICATION GUIDE INDEPENDENT DOUBLE CHECK Primary nurse and secondary verifier (RN, MD, DO or PA)completes these steps independently ►Verify patient, name of medication and dosage against the order ►Ensure use of correct mathematic calculations for preparing/mixing ►Verify dosage strength/concentration ►Verify correct route of drug administration ►Verify rate of infusion, if applicable ►Verify that IV bag is labeled appropriately and High Alert label applied ►If patient has more than one line (i.e., IV, feeding tube) mark the medication line with the “High Alert Medication” label TIME OUT AT THE BEDSIDE Requires primary nurse and secondary verifier (RN, MD, DO or PA) ►Verify patient identity (using two sources) ►Ask the patient to confirm accuracy of ID band ►Verify dose, route and pump setting of IV rate ►Review medication details with patient such as: name of medication, dosage, delivery method, etc. HAMP Policy and Procedure is located on the Ohio Intranet > Policies > under Ohio Joint Regional policies > Patient Care > High Alert Medications (JR. PC. 10) HAND- OFF Independent double check and time out must be done when transferring responsibilities at mealtimes or between units.
