May have a core committee (chair) but include others whenever possible Think about criterion for selection chair(s) Take the time to communicate Keep meeting focused on quality care (other structures for grievances etc.)

Structure: Facility Level Identify system problems and solutions Also Provides “representative” feedback on other operational and patient care issues Build from chairs of the unit level committees Coordinates work and learning between unit-based groups Insure that measure and documentation activities are taking place Include highest level of decision making possible.
