  3. Goal • Standardized approach • Promote consistency • Effectiveness of surveys

  4. State Responsibilities • Conduct one survey per worksheet before 9/1/12 • Picked 3 hospitals from list provided by CMS • Hospitals with the highest risk-adjusted readmission rate compared to other hospitals in the state.

  5. Onsite survey • 2 days on site for each worksheet. • Reports written for each survey • No plan of correction was submitted

  6. What next? • Worksheets have be revised • Will continue to be test this year • New surveys will use all three worksheets in one hospital survey.
